What is Vision Insurance?

A health and wellness plan designed to reduce your costs for routine preventive eye care including eye exams and prescription eyewear (eyeglasses and contact lenses).

Plan Rules:

Eligibility: Eligible employees, as described on page 1 of this benefits guide, spouse, & dependent children (up to age 26)
Coverage provided by MetLife
Claims must be submitted within 90 days of service
